Ця вроджена вада характеризується тим, що в період ембріонального розвитку два зачатки майбутніх нирок зростаються посередині нижніми полюсами і, в результаті, формується одна нирка, що у вигляді підкови розташовується в заочеревинному просторі над найбільшими судинами організму — аортою та нижньою порожнистою веною.
Анатомічною особливістю таких нирок є аномальне кровопостачання. Кожна половина такого органу має своє власне кровозабезпечення, але якщо в нормальної нирки зазвичай є одна артерія, то в цьому випадку їх може бути від 2-3 до 5-6, і відходити вони можуть не лише від аорти, але і від її розгалужень та брижейкових судин кишківника.
Вада зустрічається з частотою приблизно в однієї людини з 500, здебільшого у чоловіків.
Майже у 60% випадків підковоподібна нирка поєднується з вродженими вадами других органів. Анатомічні особливості цієї аномалії можуть привести до таких ускладнень, як пієлонефрит, гідронефроз, сечокам’яна хвороба, артеріальна гіпертензія.
Причиною гідронефрозу саме у Платона була компресія (стискання) лівого сечоводу великими судинами (артерією і веною), що живлять відповідний сегмент нирки, а це значно ускладнювало оперативне втручання. Здавлення сечоводу привело до затримки сечі і різкого розширення порожнистої системи лівої половини “підкови” та витончення її паренхіми. До того ж у хлопчика з’явилися перші прояви сечокам’яної хвороби.
Пересікання таких судин привело б до інфаркту (некрозу) частини ниркової паренхіми і , як наслідок, до різкого підвищення артеріального тиску у дитини. Тому під час операції сечовід було пересічено і приєднано новим анастомозом до безсудинної ділянки ниркової миски без пошкодження причинних судин.
Родина з Хмельницької області. Платону – 8 років. Батьки звертались із сином до різних лікарень України, однак всюди їм відповідали, що не можуть допомогти при такому діагнозі. Погодились зробити втручання лише у київському “Охматдиті”, однак це було якраз перед ракетною атакою на шпиталь і операцію довелось скасувати.
